Leczenie paciorkowcowego zapalenia gardła za pomocą antybiotyków skraca czas utrzymywania się objawów (o ok. 24 h) oraz zmniejsza odsetek powikłań, choć nie wszystkich. Potwierdzono, że właściwa antybiotykoterapia zmniejsza ryzyko rozwoju powikłań ropnych, w tym ostrego zapalenia ucha środkowego, zapalenia zatok przynosowych, ropnia okołomigdałkowego, a także gorączki reumatycznej, ale już nie kłębuszkowego zapalenia nerek. Bezwzględne zmniejszenie ryzyka rozwoju wymienionych powikłań było małe, dlatego skuteczność antybiotykoterapii jest słabo widoczna w bogatych krajach rozwiniętych, gdzie powikłania anginy paciorkowcowej występują rzadko.
